The following forms may be filled out online and printed.
Please complete and turn into the Tiger Card Office.
| Account Closure/Adjustment Request
This form is for students needing to close or adjust funds on the Tiger Card. You may be graduating, transferring, or needing a bookstore refund. |
| Meal Plan Exemption
This form is for students who are applying for Meal Plan Exemption or Reduction of a Meal Plan. Meal Plans are required for all first year students living on campus. |
| Payroll Deduction
This form is for faculty and staff of LSU that want to payroll deduct a certain dollar amount to their TigerCASH account. |
