Prudential Term Life
- Two different plans to choose from: the Basic Plan & the Basic + Supplemental Plan
- Flat rate of $.50/$1,000 of coverage for both plans
- Basic Plan: $5000 of coverage
- Basic + Supplemental Plan: Coverage is 1.5 times your salary. The maximum amount of coverage is 1.5 times salary up to $50,000
- Can continue into retirement at same rate
- Employee/family member needs to contact the Benefits Service Center to file a claim
LSU System Voluntary Term Life Insurance Policy
- Elect up to 5 X annual salary up to $500,000 if you enroll within 30 days of date of hire
- Premiums are age-rated and will increase according to age
- Can elect coverage amounts for spouse and children. Note : It is the employees responsibility to notify the Payroll Department to stop deductions when the youngest child is no longer eligible.
- Employee/family member needs to contact the Benefits Service Center to file a claim

Eligibility
Any active employee of LSU is eligible for life insurance provided the following:
- Employed at 75% of full-time effort or greater (at least 30 hours per week);
- Appointed for a duration of at least one semester or 121 days or greater
Enrollment
New Hire/Newly Eligible Employee-Employees must enroll within 30 days of hire or effective date of when employee became eligible for benefits. Benefits are effective the first of the month following employee's first full calendar month of employment or effective date of when employee became eligibile for benefits.

Existing employee/family member- If an employee did not enroll in coverage within 30 days of their eligiblity to obtain benefits, they will have to undergo review of their medical history with ING before approval for coverage. Employees can apply for coveage as a late applicant at any time throughout the year. Benefits will be effective once ING approves their coverage.

Terminating Employment
If the employee looses coverage or leaves LSU employment the employee/family member will be able to port their coverage for Prudential and convert their LSU System Life insurance. Prudential's rates will change because coverage is being ported. LSU System Life's rates will remain the same as active employees. The employees has 30 days from when they loose coverage to port or convert their coverage.

RETIREES
- Retirees are able to continue their life insurance coverage for themselves and their eligible family members through retirement at the same premium rate as an active employee.
- Coverage will reduce for both life insurances at age 65 and 70.
- Prudential life insurance is automatically continued through retirement.
- Retirees will have to complete the conversion form to continue the LSU System Life Insurance within 30 days of retirement.
- Prudential premiums will be deducted from LASERS and TRSL retirement checks but will be billed by the LSU payroll office if they are retired from an ORP.
- Retirees will be billed for their LSU System Life Insurance premiums every three months regardless of what retirement system they retired from.
