The following forms are due to Greek Life. You may access them on-line and turn them in to Greek Life, 472 LSU Union, once completed. Listed due dates without an attached form can be completed in Greek Life.
2009-2010 Greek Parking Form.
Event Registration Form
To host an event on campus, your organization must complete this form and return in to 472 LSU Union for approval.
Alcohol, Catering, & Security Approval Form
Student organizations that wish to have alcohol at events are required to also provide food. An LSU licensed caterer must be used for both food and alcohol. Events with alcohol
also require security from LSU Police Department. Return forms to 472 LSU Union at least three (3) working days prior to the event.
Fundraising Permit
The fundraising permit allows you to hold a fundraising event on campus. This event must be pre-approved by Greek Life and the financial statement must be turned into the Dean of Students ten days after your event.
 
Officer Verification Form
Attached if the officer verification form. When new officers are elected please turn in this form so disciplinary staus can be checked. Officers on any University disciplinary probation are ineligible to hold an office.
Chapter Update Form
Please submit this form when there ar any changes to chapter president, advisor, university advisor, house corporation president and house director.
IFC Roster Aditions
Initiation Verification
Please submit to the Greek Life Office, 472 LSU Union, no later than September 18, 2009 (Fall) or February 26, 2010 (Spring). This form describes pre-initiation activies and provides the dates of formal pledging and initiation.
Grade Release of New Members
The grade release of new members form, is to be completed by the chapter president. This form is to be turned in within 7 days of formal pledging.
Hazing Statement
The hazing statement is to be completed by the chapter president. By completing this form, the chapter president attests he or she has made all active and new members aware of applicable hazing policies. Due September 18, 2009.
Fire Drill Form
Due September 18, 2009.
House Modification Form
Housing Status
House status forms for the Fall semester must be completed by the following dates: for Fall Break (September 18, 2009), Thanksgiving Break(November 13, 2009), and Christmas Break (December 4, 2009).
