Forms

Depending on the type of service-learning project your organization is involved in, you may find the forms listed in the drop-down menu at left be useful to you. Please discuss them with your faculty partner and if you mutually agree, you and your faculty partner may download and rewrite or adapt them to fit the needs of your organization and/or your faculty partner.

Here’s a brief description of each of the forms:

Faculty/Community Partner Service-Learning Agreement may help you and your faculty partner establish the conditions of your partnership.
Evaluation of LSU Service-Learning Student may be required by the faculty partner to evaluate students near the end of the service project.
Student Service-Learning Project Log may be useful for monitoring student progress in an ongoing placement project.
Service-Learning Student/Community Partner Agreement will help you and your student partner establish the conditions of your partnership at the beginning of the semester.
